Criteria

  • Scholar applicant must manifest a participation in church liturgical services
  • Must possess catholic virtues
  • Financially in need as evidence by the Foundation’s financial questionnaire
  • Willing to serve in the parish and commit to person’s self to serving others

Requirements

  • Must submit letter of request
  • Fulfill application form
  • Schedule Interview
  • Present proof of financial incapacity
  • Endorsement of priest/religious
  • Schedule home visitation
  • For old scholars, must submit parish volunteer forms and semestral grades

(Scholarship grants are evaluated on a semestral basis and may be discontinued if scholar does not fulfill the requirements.)

2. Livelihood Assistance

Categories

  • Family/Individual
  • Institution

Criteria

Family/ Individual

  • Family could not survive daily needs and the total family income is below the acceptable per capita income of the Foundation
  • Viability of the program

Institution

  • The religious congregation /institution must submit the criteria and list of their beneficiaries
  • must show the positive effects to the community
  • Viability of the program
  • Established support mechanism to assure the success of the livelihood program

Requirements

  • Must submit letter or request
  • Must accomplish application form and submit proof of financial incapacity
  • For institutional assistance, proponent must submit project proposal form, detailed cost of expenses, endorsement of bishop and audited financial reports
  • Assessment of viability of livelihood project proposal
  • Schedule of site visitation

3. Crisis Intervention Assistance

Criteria

  • The family has experienced an emergency or crisis that has disrupted their capability to function normally and the family has no resources to recover from the crisis

Requirements

  • Must submit letter or request
  • Must accomplish application form and submit proof of financial incapacity
  • Must submit pertinent documents to justify authenticity of need

4. Disaster Rehabilitation/Calamity Assistance

Criteria

  • The community has experienced an emergency or crisis that has disrupted their capability to function normally.
  • The community is in need of external assistance in order that it could survive the immediate effects of the disaster (relief)
  • The community over a prolonged period of time is in need of resources to begin their livelihood as well as to reconstruct their shelter and community (rehabilitation)

Requirements

RELIEF ASSISTANCE

  • The Bishop/Diocesan Social Action Director/Priest must call the LBNFI Director for immediate Disaster Relief request
  • The request will be coordinated with the Archdiocesan Disaster Response committee for assessment
  • After relief operations, the Diocese must submit a liquidation report

REHABILITATION ASSISTANCE

  • The proponent must submit letter of request
  • Submit accomplished form and submit pertinent documents
  • Classify if rehabilitation assistance falls into livelihood, construction, capital expenditures or resiliency building education
  • For livelihood requests, refer to livelihood assistance requirements
  • For capital expenditures requests, refer to Crisis Intervention requirements
  • For resiliency building education requests, refer to formation and evangelization programs requirements
  • For construction requests, refer to guidelines for funding

5. Non Disaster Related Construction Assistance

Criteria

  • The community lacks financial capacity
  • The project must be essential to the needs of the community
  • The community belongs to the sub-urban and rural area with limited resources
  • Established support mechanism to assure the success of the construction

Requirements

  • Submit letter of intent
  • Must submit accomplished application form, construction plans including design analysis, structural design plan, electrical and sanitary design plan, cost estimate of bill and materials with labor cost, contract, project management schedule, government permit and licenses, photos and documentation if on-going construction
  • Submitted requirements of the proponent will be evaluated by the Consultant Engineers of the Foundation

6. Formation and Evangelization Program Assistance

Criteria

  • The community lacks financial capacity to support and fund the program
  • The project must be essential to the current needs of the community
  • The community belongs to the sub urban or rural areas with limited resources

Requirements

  • Must submit letter of intent
  • Proposed seminar/activity/program with detailed budget costing
  • Must manifest counterpart for the project
  • If sponsorship related, submit invitation or program with detailed budget costing
  • Criteria and list of beneficiaries must be provided

7. Institutional Charitable Assistance

Criteria

  • The project must be essential to the current needs of the community for a specific purpose
  • Institution lacks financial capacity
  • Must support numerous beneficiaries
  • Project must contribute to the spiritual, social and economic growth of the community

Requirements

  • Must submit letter of intent
  • If capital expenditure related, justification for use and canvass
  • Proof of financial incapacity to implement such programs
